Employer group dental and/or vision plans are offered to employers in Kentucky, Indiana and Ohio companies with two or more employees. Dental and vision coverage may be purchased together or separately.

Dental Coverage for Individuals

Our network access plan is different than employer coverage. The plan is tied to a network of participating dentists who have signed contracts and agreed to accept certain fees for dental services. Members must be located in Kentucky, Indiana or Ohio (Lucas & Wood Counties only).
