ProMedica Pharmacy Medication Management

ProMedica Pharmacy Medication Management is comprised of specially trained clinical pharmacists and support staff that work closely with you and your provider to give you dedicated management of your medications. Together we will manage your medications by making sure you receive effective treatment, routine monitoring and ongoing education.

Our clinical pharmacists will:

• Answer any questions you may have about your medications.
• Verify your medications are safe to take together.
• Review your lab values and medical history to ensure safe medication use.
• Resolve any side effects that you are experiencing.
• Determine if there are cost-saving opportunities.
• Provide education to you and your family, including:
  • Review each medication's dose, frequency, route, technique, and reason for taking.
  • Review your over-the-counter medications, supplements, and herbals.
  • Provide you with an updated medication list during the visit.
• Discuss ways to help you remember to take your medications.
• Discuss how to decrease your risk of falls if taking certain medications that can contribute.
• Help to quit smoking.
• Provide weight loss support.
• Offer vaccine recommendations based on your age and medical history.
