In Person: Get an application form at the registration desk at any ProMedica location. For inpatient services, a Patient Financial Advocate can help you complete and submit an application.
Request by Phone: Contact Customer Service at 844-373-0871 (Monday through Thursday, 8 a.m. – 7 p.m. and Friday, 8 a.m. – 6 p.m.) to have an application mailed to you.

Once we receive your application and documentation, we'll notify you by mail of approval or denial.
If you need help managing your medical bills over time, ProMedica also offers flexible payment plan options.
For balances that require more than 12 months to pay, ProMedica partners with CarePayment to offer:
CarePayment is a partner used by health systems across the country to help patients manage medical expenses.
Financial assistance and payment plans are separate options. If you qualify for financial assistance, you may receive free or reduced-cost care. If you still have a remaining balance, you may be eligible for a payment plan through CarePayment.
