For more than 10 years, ProMedica has pioneered this effort and has become an industry leader in thought, innovation and action in addressing the social determinants of health (SDOH). We see an unprecedented opportunity to improve health outcomes and reduce healthcare costs. Through the ProMedica National Social Determinants of Health Institute, we're driving local and nationwide change in how healthcare is delivered.
When most people think of healthcare, they tend to think of clinical or medical care, such as going to the hospital or doctor’s office. But medical care only accounts for 20 percent of a person’s health and well-being. The other 80 percent is affected by environmental, behavioral, social and economic factors.
The social determinants of health are conditions in the environments in which we are born, live, learn, work, play, worship and age. They affect a wide range of health issues, risks and outcomes. They also impact a person’s quality of life.
At the ProMedica National Social Determinants of Health Institute, our mission is to create healthier people and communities. To do this, we establish local, regional and national opportunities to integrate social determinant factors with clinical care. This provides a more holistic approach to health and well-being.
We're grounded by our strong community ties with healthcare providers, payors, nonprofits, corporations, civic leaders and the people we serve. Because of this, we research, test, shape and invest in innovative solutions to improve the health of everyone in our region.
At the core of our SDOH initiatives are the people we serve. We take a thorough look at what makes individuals healthy, including their lifestyle, food security, financial stability and access to employment training. We also help create safe neighborhoods, inviting public spaces and thriving communities.
ProMedica began treating hunger as a health issue in 2009. From there, our work expanded to have an increasingly broader scope as we’ve taken on more challenges to address the social determinants of health.
We’re leading a national focus on SDOH to redefine healthcare and create a new model that invests in public health, invites national and local partnerships, and implements interventions that affect the many factors that affect individual and community health.

As a not-for-profit health and well-being organization, philanthropy allows us to focus on community needs through projects and initiatives we could not otherwise do. Named for the late philanthropist Russell J. Ebeid, the Ebeid Neighborhood Promise is a national model for neighborhood revitalization that uses a place-based approach. The initiative focuses on jobs, education, health, stability and resident engagement.
Dedicated to increasing access to healthy food, delivering nutritional education and providing job training opportunities, ProMedica Ebeid Center features a full-service grocery store in a former food desert. The second floor offers financial and employment coaching, and community programming such as cooking and nutrition classes, CPR courses, fitness classes and health screenings.
