There are many things that impact your health your job, your education, your lifestyle, and even your zip code. While clinical and medical care make up about 20% of your health, the other 80% is affected by your environment, behaviors, social and economic status. These are called the social determinants of health (SDOH).
At ProMedica, we know that every individual has experiences that shape who they are and how they manage their health. We take a holistic approach to well-being by addressing these factors as well as working to improve the health of our communities.
When you're a ProMedica patient, we see our encounter with you as an opportunity to not only provide high quality medical care, but to help identify other factors that may be affecting your health outside of our hospital or office.
That's why we screen patients for social determinants of health each day throughout our health system. These screenings are part of regular exams and care in our hospitals. Our care team may ask you about:

As part of our SDOH screening, we ask our patients two questions regarding regular access to food. If you screen positive for food insecurity as a patient in a ProMedica hospital, we provide a basic food care package at hospital discharge along with information about community resources. If you screen positive at a ProMedica doctor's office, we connect you with ProMedica's food clinic, where you'll receive several days' worth of food for your family, access to nutrition counseling and other community resources.
Your financial stability plays an important role in your health and quality of life. ProMedica is the first health system in the nation to operate a Financial Opportunity Center (FOC), located at ProMedica Ebeid Center. We offer free financial coaching in a group and one-on-one setting to people of all zip codes and incomes. Financial coaching helps individuals acquire the skills to budget and save, develop healthy banking relationships and raise credit scores.
"We offer jobs and employment training through ProMedica Ebeid Center, both through our job training program at Market on the Green and through community classes. This includes a 5-week State Testing Nursing Assistant (STNA) training program to prepare students for providing resident/patient care in multiple healthcare settings, such as long-term care facilities, hospitals and home health agencies.
ProMedica also hosts a Summer Youth Employment program for individuals ages 16-22 who are interested in healthcare careers and professional development. The program is in partnership with Jesup W. ProMedica partners with Toledo Area Humane Society, local mental health providers and the University of Toledo to leverage the benefits of human-animal interaction to improve mental health and wellness in individuals. Homeless pets are placed with individuals who are referred to the Hope and Recovery Pets Program (HARP).
Recognizing the effect that social isolation has on well-being, ProMedica partners with community organizations to connect seniors with program volunteers trained in social isolation and increasing awareness of community resources. Individuals receive weekly calls and quarterly newsletters during the 12-week program and can choose to continue calls after the program ends.

ProMedica partners with The Arts Commission to engage local leaders, partners, residents and stakeholders in comprehensive community development efforts focused on place based art and community engagement.
ProMedica partners with Goodwill's job training program, helping individuals to acquire the skills to budget and save, develop healthy banking relationships and raise their credit scores.
ProMedica partners with JumpStart to significantly increase entrepreneurial outcomes through the development of neighborhood-based small business, with the focus on women and minority owned businesses. This is achieved through small business growth collaborative, an Impact Program, and Funding Pool, with an emphasis on supporting individuals living in the Uptown neighborhood.
ProMedica is partnering with LISC (Local Initiatives Support Corporation) across Ohio on Financial Opportunity Centers, including the first-of-its-kind to be run by a hospital system at ProMedica Ebeid Center in Toledo. ProMedica and LISC also partner on the Health Impact Fund, a loan pool to kick-start revitalization projects and enterprises to ramp up inclusive growth and increased economic opportunities in urban and rural communities throughout northwest Ohio and southeast Michigan.
Pregnant women need early and regular prenatal care to help ensure their babies are born full term and at a healthy weight. Unfortunately, not all women have a support system to help care for themselves, manage stress and receive prenatal care. A partnership with The Hospital Council of Northwest Ohio, The Northwest Ohio Pathways HUB regional care coordination system, launched in 2007 to combat infant mortality in Lucas County. Since then, low-income pregnant women have been connected to the medical care and social services needed to have full-term, healthy weight babies.
