Pathways Program

With the main goal of reducing infant mortality in Lucas County; the Pathways Program provides a support system for expectant mothers by helping them care for themselves, manage stress, and receive prenatal care.

Program Services

The Pathways Program can help expectant mothers with prenatal care and preparation including:

  • Getting health insurance, medication, food, housing, transportation, utilities and other basic needs.
  • Going to doctor's visits and medical tests.
  • Understanding the health care system and using insurance.
  • Combating stress, losing weight or quitting smoking.
  • Preparing for the baby.
  • Tracking blood pressure and other health indicators.

How It Works

ProMedica partners with the Hospital Council of Northwest Ohio to train and employ certified community health workers (CHW). CHWs then work with expectant mothers to provide needed medical care and social services. CHWs conduct home visits to provide education and connect expectant mothers with a wide range of resources based on the mother's needs.
