• A treatment plan developed between you and the therapist during the initial evaluation.
• Follow up sessions, if necessary, scheduled to meet your financial and lifestyle requirements.
• Written plan of care and progress reports shared with the referring physician.
• A home program designed to address your individual needs.

How To Schedule an Appointment

Physical therapy is available at ProMedica Total Rehab locations throughout northwest Ohio and southwest Michigan.

At some of our Total Rehab locations, you may begin physical therapy services for chronic or acute pain not caused by a traumatic injury without a physician referral. View locations where self-referral is available.

For other locations, a physician referral is required to begin rehab services. Collaboration with a physician is also needed for long-term care at all locations. Please consult with your primary care physician to determine if physical therapy is right for you.

Your Connection to Specialty Care

With more than 1,200 providers in 55 specialties, our ProMedica Physicians network covers every major specialty. Our therapists will work with your physician to refer you to a specialist if necessary. Together, we’ll help you get the care you need to be the healthiest you can be.
