ProMedica Fostoria Community Hospital is a 25-bed acute-care Critical Access Hospital serving the residents of Fostoria and surrounding communities.

As one of the largest employers in Fostoria, we’re deeply invested in the economic and social health and wellness of our community. Where can I find information about hospital policies and services?


We're sure you have plenty of questions about your stay or visit at ProMedica Fostoria Community Hospital. Most of your questions can be answered by viewing our Patient Information Guide or Visitor Information Guide. However, please see the most frequently asked questions below to help you during your time with us.

Where can I park at the hospital?


There are many options to park at our facility. Please see our hospital campus map for all parking spaces.

What are visiting hours?


Visiting hours depend on the department and the patient's level of care. Ask a staff member for more information. Rest is an important part of your care, but we know that family and guests help make your hospital stay more pleasant.

Due to space limitations in some patient rooms, it may be necessary for your doctor or nurse to limit the number of family/guests in the room. Requests for visitors to stay overnight are assessed on a case-by-case basis and require approval of department leadership.

Quiet time is observed daily from from 2 – 4 p.m. and 10 p.m. – 6 a.m. During those hours, we ask that you and your family/guests help us maintain the quiet, peaceful atmosphere necessary for our patients' well-being and healing.

Who will take care of me in the hospital?


You may see more than one doctor while you are in the hospital. The doctor who admitted you may seek consults from other doctors about your care. All employees wear picture ID badges that include their name, title and department. You may encounter a number of employees during your stay.

Is there an ATM in the hospital?


No, there is not.

Where can I eat?


The Serving Spoon, the cafeteria at ProMedica Fostoria Community Hospital, is located on the first floor and features a variety of options. The café is open every day from 7 a.m. - 5:30 p.m. Simply To Go options are always available.

Everyday Mobile Ordering is available on the Everyday App. You can download the app on your phone through Google Playor the Apple Store. 

The Serving Spoon is cashless. Only credit or debit card payment is accepted. For your convenience, vending machines are located next to the gift shop on the ground floor. 

 

Do you provide internet and television services?


Wireless internet access is available throughout the hospital. Use the “PHSguest” network on your wireless devices. If you have difficulties connecting, call ProMedica’s Help Desk at 419-291-3646.

TV service at ProMedica is free of charge to patients. Channel guides are available in each room.

 

Does the hospital have a gift shop?


Yes! The Gift Shop is located on the first floor of the hospital, directly inside entrance 3. The Gift Shop offers a variety of home décor, jewelry, books, toys, greeting cards and snacks.

The gift shop is open Monday – Friday from 9 a.m. – 3 p.m. Cash, checks and credit cards are accepted. Free gift wrapping is available.

Is my patient privacy protected and secure?

What do I do when I have a concern?


We truly hope you are pleased with your care and your stay with us. If for any reason your concerns or needs are not addressed, please request to speak to the unit director or their representative.

Additionally, your loved ones may have concerns that need to be addressed. They are more than welcome to bring them to the attention of your caregivers. You or your loved ones can also submit concerns online or leave a message on our dedicated phone line, 419-291-4000.

Who do I contact if I have billing questions?


If you have questions or concerns about paying your hospital bill, please contact our financial assistance department at 800-477-4035 or 844-373-0871, Monday – Friday during normal business hours.

Where can I find estimated costs of hospital services?


You have the right to understand your services and the costs associated with those services. For your convenience, we provide listed price transparency resources for each of our hospitals. Please note that these estimates are for the most common services offered at that facility and do not show your insurance coverage or other variables.

What if I need more care after I leave the hospital?


ProMedica offers many options for post-hospital care. Your doctor, along with our staff of social workers, nurses and care navigators, will connect you with services to help you continue your recovery following your hospital stay. If you would like a list of facilities that serve our geographic area, please ask your nurse.

How can I recognize a hospital caregiver?


We’re so pleased to hear about your wonderful experience. Our caregivers are the backbone of our hospital, and we want to make sure they receive recognition!

The DAISY Award is presented to a deserving nurse to celebrate the extraordinary compassion they provide patients and families every day. Through this award, the hope is that each nurse who receives the award truly recognizes the positive impact they have on the patients in their care. We encourage you to nominate a nurse who went above and beyond for you during your stay.

The ROSE Award was established in 2020 by the Toledo-area ProMedica hospitals to recognize outstanding employees in all roles in the hospital. This award celebrates the extraordinary efforts of non-nursing employees who provide outstanding patient services in any position. We encourage you to nominate someone who you feel exceeded expectations to make your stay pleasant.

Anyone can nominate a nurse or ProMedica employee for a ROSE or DAISY Award! At the end of each quarter, the hospital will announce one winner of the ROSE Award and one winner of the DAISY Award.

How can I make a donation on behalf of a loved one?


The Grateful Patient Program is an enterprise-wide initiative that includes all 10 ProMedica hospitals along with our ProMedica Physicians. This program provides an opportunity for individuals served by ProMedica to say thank you for the exceptional care they received by sharing their story or making a financial gift of impact.

The program also includes the opportunity for individuals and their families to honor or recognize a caregiver/care team. If you are grateful for the care and compassion you or a loved one have received from ProMedica, consider giving a donation in honor of a special caregiver in your life and they will be recognized with a champion of philanthropy pin.
