All ProMedica Physicians primary care providers allow patients to schedule their annual wellness visit through MyChart. Additionally, many of our providers and specialists have online scheduling for follow-up appointments.
If you’re an established patient with a ProMedica Physicians provider, scheduling an appointment is easy! All you need is a smartphone, desktop or tablet to get started.
Log into the ProMedica MyChart website or mobile app, then click “Schedule an Appointment” from the top navigation. For more information, download detailed instructions.

Two days before your appointment, you’ll receive a message letting you know it’s time to complete eCheck in. Log in to ProMedica MyChart and select the eCheck-In button by your appointment notice on the home page. Then, answer the questions and complete the online forms.
Once you've completed all eCheck in and registration steps in ProMedica MyChart, you'll be eligible for self arrival. Self-arrival means using text messaging or MyChart to let us know you've arrived for your appointment. When you self-arrive, you don't need to register at the desk – you can head directly to the waiting area.
You can complete self arrival through a text message or by using the MyChart app.
Five minutes before your arrival time, you'll receive a text message asking you to let us know when you've arrived. When you're inside the building, reply to the text message to confirm your arrival.
Please note that you must first approve text messages from ProMedica to use this feature.
To activate self-arrival within the MyChart app, turn on location services on your device and enable notifications in your MyChart account. (You may also need to enable notifications in your device settings.) Enable appointment arrival by going to MyChart's “Account Settings” and reviewing the Appointments Arrival setting.
Five minutes before your arrival time, you'll receive a MyChart app notification asking you to let us know when you've arrived. When you're inside the building, follow the prompts within the app to confirm your arrival.
Once you've self-arrived by text or MyChart, your arrival will be automatically confirmed. You can go to the waiting room or the waiting area nearest to your appointment location. A team member will be with you when they are ready to start your appointment.
If you don’t have an active MyChart account, you can request activation online, ask to sign up during your next office visit, e-mail us at MyChartSupport@promedica.org, or call our MyChart Support Line at 419-291-4173 or toll free at 1-833-212-1586 Monday through Friday, 8 a.m. - 6 p.m.
If your established provider doesn’t appear when you try to schedule through MyChart, you will need to call your provider’s office. You can use our online Find a Provider tool to find your ProMedica provider and call their office.
You may be able to see your provider sooner by using MyChart’s wait list feature. Select “Get on the Wait List” for your upcoming visit.
To be notified when a wait list time is available, go to the three-line menu button and select "Communication Preferences" from the drop-down menu. Under the "Appointments" section, you can update your wait list offer settings to receive a notification when an earlier appointment time is available.
Once an earlier appointment becomes available, the top five patients on the wait list will receive an email alert and/or push notification with an offer for a new appointment. Wait List offers expire and are first come, first served.
To view a wait list offer, log in to your ProMedica MyChart account. On the homepage you will see a notice, showing your current appointment as well as the wait list time being offered. If the new earlier time works for you, just click the green, "Accept New Time" button at the bottom, and you're all set!
If you do not accept a new appointment offer, you will keep your original appointment, remain on the wait list, and be notified in the future if additional appointment times become available.
Yes, you can make an appointment with the family member’s established provider if you have proxy access. Proxy access allows a parent, guardian or loved one to log into their personal MyChart account, and then connect to their loved one’s information.
If you have proxy access, log into your personal MyChart account, then switch to view your family member’s account. Follow the same steps outlined above to schedule an appointment online.
If you do not have proxy access, you can send a completed ProMedica MyChart Proxy Access Request Form by email or fax (419-479-6919), or drop off the form at any ProMedica facility. Please be aware that information sent by email is not secure and could be misdirected or intercepted in transmission.
